Firstly, you can consider as below configuration.

1) Go to T-Code : OLMB and here create the general reason code for material adjustments such as

                          poor quality or some other reasons.Or you can define the record reason for the each

                          Goods movement type.

p.s.) If you do not want this fields as mandatory, make that one as optional field in screen layout

       of MIGO.
